Doctor Who All-Consuming Fire Annotations; Prologue & Chapter One
Prologue
The Old Man, his granddaughter and the British Army officer are the First Doctor, Susan, Siger Holmes respectively. The Doctor and Susan originate from, whodda thunk it, Doctor Who while Siger Holmes is a direct lift from Baring-Gould’s Sherlock Holmes of Baker Street: A Life of the World's First Consulting Detective.
Lane takes a number of elements of Baring-Gould’s attempt at a biography for Holmes, including both Siger and the third older, older Holmes brother, Sherringford, who will appear later in the novel. Gould identifies Siger as the Holmes family patriarch and seemingly derived the name from the detective’s use of Sigerson as an alias in The Adventure of the Empty House.
The other figures present during the prologue are of course the Seventh Doctor, Ace and Bernice Summerfield following the events of the book. They primarily serve to add a few tantalising hints at what’s to come and help introduce Lane’s notion within the book that Holmes and Watson are false-names attributed to the duo by Doyle during the publication of Watson’s memoirs. An idea largely abandoned by the time they appear in Happy Endings.
With Barbara and Ian notably absent during this sequence, it could be taken that the events are pre-Unearthly Child. However, you could always assume that they are just around the corner and aren’t terribly interested in Siger’s tale. There’s a slightly indulgent vibe to the entire set-up, however, Siger’s knowledge of the temple ultimately plays a larger role within the novel and beyond that, there’s a nice atmospheric element to these figures existing on the periphery of the tale.
Chapter One
“thirty-five volumes of my diary” - Doyle wrote 60 Holmes stories between 1887 and 1927, however chronologically his adventures begin in 1881, All-Consuming Fire takes place in 1887, with The Final Problem occurring four years later in 1891. The later is foreshadowed throughout the book, and one of the short’s more notable figures will appear in a minor role later in the novel.
“I see the repulsive story of the red leech, the terrible death of Crosby the banker.”- Lane continues Doyle’s trend of hinting at untold Holmes stories within Watson’s diary. The line paraphrases a similar moment inThe Adventure of the Golden Prince-Nez. Numerous pastiches have attempted to document these cases to one degree or another, and Lane himself would subsequently use the red leech for his second Young Sherlock Holmes novel.
“The singular affair of the aluminium crutch and its connection with an attempt upon the life of our dear sovereign…” - Another untold tale, this time lifted from The Musgrave Ritual. Its role in an attempt on the life of Good Queen Vic is seemingly an addition by Lane, and I can’t help but see this as a reference to the Jackal’s use of an aluminium crutch during his attempted assassination of Charles de Gaulle during the final act of Frederick Forsyth’s The Day of the Jackal. “Following the tragic curtailment of my marriage to Constance Adams of California I was again living under the same roof as Holmes.” - The exact number and nature of Watson’s wives are a running joke/source of hilariously serious debate within Holmes Fandom. Born of a few off-hand mentions and Doyle clearly not giving a fuck, Watson seemingly goes through between two to seven marriages. Gould alleges Constance Adams was set to be Watson’s bride to be in Doyle’s unpublished play The Angels of Darkness, so this is a bit of a twofer reference. The failure of the marrage is a bit of a joke on this front.
“The cost, he claimed, was of no concern, for he had recently been generously remunerated by Lord Rotherfield for proving to the satisfaction of various Coury circulars and scandal sheets that Lady Rotherfield was not a female impersonator.” - Another Untold Tale, seemingly a Lane original this time and an unnecessarily unpleasant “joke.”
“Finally, completely restored to health and happiness, we returned to England on the Orient Express.” - While the Orient Express was a real long-distance passenger train, it’s hard to image Lane didn’t leap at the chance to have the two return to England via the service for obvious reasons. “...Colonel Warburton and his charming wife Gloria.” - Presumably, the same Colonel Warburton whose supposed madness would come to the attention of Holmes via Watson. One of two such instances mentioned in The Engineers Thumb.
“..but only the Reverend Hawkins was present in the dining car. Baden-Powell, a self-proclaimed expert on butterflies whose tan and manner indicated military service, was absent.” - Hawkins is seemingly a Lane original, however his alias shares a surname with Doyle’s first wife. Baden-Powell is presumably Robert Baden-Powell, founder of the Boy Scout Movement and at the time of the novel Intelligence Officer within the British Army. Powell often travelled disguised as a butterfly collector and would use detailed drawings of butterfly wings as a means of hiding maps and other sensitive information. His presence during the sequence adds a touch of humour to Holmes’ complete failure to pick up on this while noting Hawkins own subterfuge.
“The man in the chair, swamped by his white robes, was the least impressive thing in the carriage.” - As we’ll soon learn, this apparently unimpressive figure is, in fact, Pope Leo XIII, who served as head of the Catholic Church between 1878 and 1903.
“I am Cardinal Ruffo-Scilla, and this,’ he gestured to his mirror image on the other side of the chair, ‘is Cardinal Tosca.” - Cardinal Tosca’s sudden death will latter be investigated by Holmes in yet another untold adventure mentioned in Black Peter. Notably, as with the events of All-Consuming Fire, this is at the behest of the Pope. Ruffo-Scilla is an odder figure, sharing the name with a real Cardinal. However, the Ruffo-Scilla died around three decades prior to the events of the novel. I’m tempted, primarily for fun anagram reasons, to view him as yet another Scaroth splinter particularly as it adds a few of extra layers of mirroring to the scene. Who and Holmes characters on either side of the Pope and all that shit.
“‘I remember Sherringford writing to tell me,’ he murmured, ‘ that one of our distant ancestors had been Commander in Chief of the Naval Forces of his Holiness the Pople. I had never credited the story until now.” - The first overt mention of Sherringford within the novel, this also takes another element from Gould in that the Holmes family are seemingly lapsed Catholics. Watson’s surprise at Holmes’ sudden, casual, revelations regarding his family recalls his shock upon first meeting Mycroft in The Adventure of the Greek Interpreter. “Have you heard of the Library of Saint John the Beheaded?” - Recurrent minor Who fixture that first appears here, the Library holds a number of rare, banned texts. Lane would subsequently detail elements of it’s founding in Empire of Glass. Where rather fittingly, the Doctor’s alleged older sibling Irving Braxatiel played a role in its creation.
“One of the three unexpurgated versions of the Malleus Maleficarum is in the Library,” - The Hammer of Witches, Well known treatise on Witchcraft that encouraged the extermination of its practitioners. Written by discredited clergyman Henrich Kramer, lots of blatant insights into the mind of a murderous wanker.
“...along side shelves of books on the Chinese Si Fan society and its leader, Doctor Fu Manchu - a man whom we in the Vatican believe to be as huge a menace to civilization as you believe anarchism to be.” - Fu Manchu is the creation of Sax Rhomer, appearing in 12 novels between 1913 and 1948. Manchu is the archetypical yellow peril, inspiring countless equally racist figures including Who’s own Li H'sen Chang. An Anti-British figure, Manchu would battle cheap Holmes knock-off Dennis Nyland-Smith in an attempt to end British Imperialism. Rhomer was a joyless fuck, so this was treated as the Doctor’s greatest crime. Lane portrays the Si Fan as a largely unknown force during the late 18th century, and this fits quite well with the early 20th century setting of the Manchu novels.
“The Affair of the Politician, the Lighthouse, and the Trained Cormorant” - Title of an episode of The New Adventures of Sherlock Holmes radio show, context would suggest Lane’s version is slightly more salacious.

Welcome to Kellerman’s
Holiday camp seasons were good business, good money and exactly what he needed to survive. His beat up old car carried him, Regina and his little boy on up just as it did every year. He and his odd little family - Regina was his closest and oldest friend, the two of them partners and bouncing around the coast together for over 10 years as they took their dancing on the road - and his little boy Bae was his world. They always arrived at Kellerman’s around two weeks before the season started so that they could practice, get accustomed to any changes at the premises - there never were any, though - and so they had a roof over their heads at the staff quarters.
Summer was always good business for them with people of all ages wanting to flock to Kellerman’s for a few weeks of rest, relaxation and entertainment. It meant a lot of people wanting dance lessons and a lot of people to watch them dance. They were paid a good enough wage to keep them going until the Christmas season when they returned. Any money they did have went on renting a place together when they didn’t have accommodation, making sure a roof was over their heads. Years before they’d often sleep in the car but with him having a three year old boy, four walls and a roof were a necessity.
Between Christmas and the Summer they found odd jobs at odd places, performing and dancing for decent money. It wasn’t an easy life but it was the one they’d fallen in to and found each other in.
Kellerman’s wasn’t exactly modern but the money was fine and the staff accommodation was good enough. The people in charge were douchebags that looked down on them but that was no surprise. Rum knew where he lay on the ladder of life, the bottom rungs that everyone stepped on on their way to the top. It was what it was, and it wasn’t something he had the power to change.
A few months in to the Summer season and his pockets were lining up with money. Performing on stage multiple times a week with Regina, getting a little cash on the side with other activities and Rum was just living his life as he did every single year. The last few years a dancing friend of theirs, Dorothy Gale, helped out with taking care of his boy. Dorothy taught classes to children during the days and during the evenings tended to help out by babysitting little Baden whilst he worked. Sometimes his other work meant Bae spent the night with Dorothy or Regina but mostly Dorothy. Regina herself had a budding relationship with someone that she shouldn’t; a waiter by the name of Robin.
In fact, she was pregnant. A pregnancy that she wasn’t entirely aware of yet even if she suspected. Simply too afraid to take the test she continued to dance with Rumford and line her own pockets. The summer of ‘63 was being kind to them and giving them decent dance classes. Regina taught a class during that day and nobody would have known the wiser about her condition.
Soon after it was time for yet another staff briefing, the kind of thing Rumford was more than sick of. He knew how to do his work, he knew how to get the guests to empty their pockets and so as he sauntered in with his jacket slung over his shoulders he just stood there and took it as the manager and his catering staff spoke down to he and his fellow performers like usual. He just let it brush off, roll down his shoulders and back till he was stomping it in to the ground. It didn’t matter, none of it did.
Which had he and Regina in the middle of the dance floor that evening, dancing a mambo that showcased just what they were capable of. Show off what lessons could get people in to thinking they were capable of. Attract people to the shows at night. Maybe even attract attention from people wanting to offer up money for other services. Either way it was part of their usual move and with a few moves too many they ended up dancing with patrons, trying to charm them so they’d end up in classes during their stay. Only time would tell if it worked.
Soon after he and Regina were returning to his cabin to say goodnight to little Bae, Robbie telling him a story to lull him off to sleep. Dorothy wasn’t one for parties so even though the entertainment staff were gathering as they did most nights she was content to stay back and take care of the boy, leaving Rum and Regina free to enjoy a few hours of dancing and socialising. A perfect way to unwind. They entered to people greeting them with wide smiles and attention, Rum and Regina soon found their natural place in the middle of the dance floor, dancing closely to each other with pure chemistry flowing between the two of them, dancing the kinds of moves that Kellerman’s would never allow on their dance floors.

15 Oct 2020 Germany’s first 3D-printed home
Germany’s first 3D-printed house is located in the North Rhine-Westphalian town of Beckum. The two-storey, 160 m2 single-family home is currently under construction.
The 3D printing process has already been applied to the walls of the house. A nozzle applies special concrete in layers. The print head moves over three axes on a fixed frame and is controlled by just two people.
It takes just five minutes to print one square meter of a double-shelled wall. This innovative technology saves more than time compared to conventional construction methods; it also significantly reduces resources and allows for greater freedom in building design.
Viessmann heating, cooling and ventilation products have been selected for this home by PERI GmbH, one of the world’s leading suppliers of formwork and scaffolding systems, as well as civil engineering solutions. When completed, the first home will be heated and cooled by a high-efficiency Vitocal 200-S air/water heat pump and ventilated by the Vitovent 300-W ventilation system.
The temperature will be perfectly regulated all year round. The heat pump is highly efficient with a COP (coefficient of performance) of up to 5.0 (EN 14511 at A7/W35°C) and has an energy efficiency rating of A++.
The new Vitoset heat pump-hybrid cylinder WPU 300/100L will be installed as a heating buffer and DHW cylinder. The hybrid cylinder solution saves a lot of space since it consists of one 300-litre enamel DHW cylinder and a 100-litre buffer cylinder. The cylinder is delivered in one piece and is completely insulated.
Fresh, clean, and especially germ-free ambient air is more important than ever in times of Covid-19, and so the Vitovent 300-W central home ventilation system will also be installed in Germany’s first 3D-printed house. This quiet ventilation system is particularly quiet and compact and recovers up to 92 per cent of the heat from extracted air during the cold weather, saving heating costs. In combination with the Vitocal 200-S heat pump, the ventilation can be conveniently controlled using the free ViCare app on a smartphone.
PERI GmbH expects 3D printing to gain in importance in the next few years, and additional residential projects are already in preparation.
With a turnover of EUR 1,685 million in 2019, PERI is one of the largest global manufacturers and suppliers of shuttering and scaffolding systems. With more than 9,500 employees, over 60 subsidiaries and more than 160 warehouses, the family business with its headquarters in Weißenhorn, Germany serves its customers with innovative system equipment and comprehensive services for shuttering and scaffolding technology.

Balancing act: software giant SAP navigates Russian risks
MOSCOW/WALLDORF, Germany (Reuters) – Most Western technology companies are losing ground in Russia under the weight of restrictive rules and mounting local competition. Germany’s SAP (SAPG.DE) is thriving.
A view shows an installation with the logo of SAP software company on the roof of an office building, with a traffic sign seen in the foreground, in Moscow, Russia April 23, 2018. REUTERS/Tatyana Makeyeva
SAP is the clear leader in the Russian business-planning software market, supplying 53 of the top 100 Russian companies by revenue, according to a Reuters analysis of company filings.
Its success has come at a time of intense legal pressure on foreign technology firms, including a law requiring them to allow Russian authorities to hunt for vulnerabilities in their software, which has raised security concerns in Washington.
While the likes of Oracle (ORCL.N), Microsoft (MSFT.O) and Google (GOOGL.O) have been losing market share, Russia is one of SAP’s fastest-growing markets, with revenue rising by about a third to 468 million euros ($565 million) last year.
Part of the reason for this, analysts say, is that its products are entrenched in running the biggest state firms in industries including energy, metals, transport and retail. The German firm has invested heavily in the market, even sponsoring top-tier Saint Petersburg football club Zenit.
It is consequently winning new work with clients, including supplying cloud-based applications that build on its existing software; for example, last month Russia’s largest bank Sberbank (SBER.MM), which is state-owned, said it had put in place a new SAP human resources system covering 230,000 employees.
“The large state companies … all use SAP because of the long-term investment involved, the money already spent and because the software works,” said Moscow-based software analyst Elena Semenovskaia of global tech research firm IDC. “You would have to be insane to rip out SAP and install something else.”
At a time of fraught ties between Russia and the West, SAP has also taken steps to smooth relations with authorities on both sides.
Russia is a small market for SAP, representing just 2 percent of its global revenue, but its actions highlight the complications facing global technology firms operating in a world divided over national security concerns. Balancing the security interests of one country with those of a rival power, when both are using the same software, can be a minefield.
For example, SAP has hired a former general from Russia’s FSB Federal Security Service to help it manage its relationship with the Russian government and security services, according to publicly available records and a source close to the company.
Slideshow (3 Images)
In a move that could help assuage Western security concerns about its exposure to Russian authorities, the company also says it only allows Russian examinations of its products’ inner workings, or source code, to be conducted at a special “clean room” laboratory in Germany.
SAP told Reuters it has complied on rare occasions with Russian software vetting rules while making every effort to ensure the security of products and customers elsewhere.
Asked about the hiring late last year of the ex-FSB general, Vladimir Vladlenovich Skorik, it said: “Such individuals may provide insight into the certification processes that SAP will have to undertake as it sells into the public sector.”
Reuters could not establish Skorik’s exact job title or responsibilities. He did not reply to a message sent to his official SAP email account.
The practice of employing former operatives to give companies insight into state security processes is also common in some Western countries, including the United States.
The Kremlin and FSB did not respond to requests for comment about Skorik’s employment.
INSIDE THE CLEAN ROOM
SAP is Europe’s biggest software company but number four globally behind U.S. rivals Microsoft, Oracle and IBM in terms of annual sales. It focuses on business-planning software while rivals are more diversified.
Microsoft faces a tough road in Russia because its accounting-focused software faces direct competition from the Russian market leader in that area, 1C. Database giant Oracle has also lost ground in the market over the past decade.
Google and Facebook have long struggled with copycat rivals – Yandex in search and VKontakte in social networking – that put them in the unusual position of second-place players. Recent antitrust and data law changes have also increased pressure on both firms.
SAP, by contrast, is the undisputed leader in the Russian business-planning software market, with 50 percent of the market.
Reuters has reported in recent months that SAP and other companies have acceded to demands by Russian authorities, including the FSB, to allow military contractors to review the source code of some of their products.
Moscow says the reviews are necessary to detect flaws in foreign-made software used by Russian state companies which could be exploited by hackers. But U.S. lawmakers fear they pose a security threat as the same software is used by American government agencies.
Cyber security firms Symantec and McAfee no longer allow foreign governments to review the source code of their products because of security concerns. This shuts them out of parts of Russia’s state technology market, but it was unclear how much business either company previously had in those areas.
Reuters was given exclusive access to visit SAP’s “clean room” near its headquarters in the town of Waldorf in the southern German state of Baden-Wuerttemberg.
Visitors descend below ground through three zones of security to enter a metal cargo shipping container equipped to block electromagnetic signals.
Russian code-testers accredited by the Russian security services fly in to conduct the examinations. No outside electronics, notepads or even pencils are allowed into the site. The testers have access to an isolated computer screen, keyboard and nothing else, SAP Global Vice President Stefan Schulz said.
Schulz said there had been a handful of Russian code reviews of SAP software in recent years, and none in the past year. No other government has carried out such reviews, he added.
He said security reviews have turned up only already known issues. “I don’t know of any case where any new vulnerability was found in the context of such a source code review,” he said.

Five months of European Voluntary Service – an honest reflexion.
Hey everybody. Today, something serious and something very personal. I’ve been writing this text for the last two weeks, added some things, deleted a lot of things and ended up writing them again, just to repeat all of that. I’ve been here in Finland for five months now and a little more than the half of my EVS is over – so I thought it’s about time to share a little more than the usual “what happened this week” stuff. What follows now is an honest and personal reflexion about the first half of my life as an European Volunteer and how that influenced me and my point of view on a lot of different things.
My adventure of EVS started on the first of September 2017. I planned to participate in the European Voluntary Service for quiet a while back then – two years. At BiondekBuehne we usually have at least one person doing an EVS every year – and almost three years ago we had a guy from Finland over. We became close friends and he was the origin of my idea to go abroad. Since then, the will to realize my idea grew stronger and stronger. I wanted more of life. Don’t get me wrong, I have a great life, an incredible family and the nicest of all friends. But I wanted more. I wanted to develop beyond the Alex Ertl I knew for 18 years. I wanted to change. Not because I didn’t like myself. More because I wanted to know more of the person that I already liked. I fought through the two last years of school (Austrian school system sucks by the way but that’s something to discuss in another post), managed to pass my final exam and then there was nothing else in my way to go to Finland. I didn’t had any doubts about what I was doing when my last week in Austria arrived. I was ready, I was so eager to finally go to this unknown country and I was curious about everything what would happen there. Saying goodbye to everyone was a price that I was willed to pay, surprisingly it didn’t even bother me that much. I had the fear to miss something in Austria during my EVS, but the need to see new things excelled that fear.
At 14:40, my flight landed in Finland. I didn’t really know what to feel at this moment, I was just extremely excited and nervous. But in a positive way. I worked for this experience. I had so many people giving me advise for everything around EVS. I felt extremely prepared. And the first couple weeks felt like a dream. I had every possibility here, the people were incredibly nice, taking care about Romain and me, showing us what kind of jobs we can do here. Couple weeks later we had the On Arrival Training, a four day long seminar, where all EVS in whole Finland come together to get to know each other and to learn more about their purpose and their possibilities. I met a lot of new people, I became friends with some. It was an experience that someone can’t progress in one day, not even in a few days. Six weeks before I was sitting in my room in Baden, writing a short story. Suddenly I was sitting in North Finland, on the roof of a watchtower, staring at the open sea, together with people from all over Europe. That was the first time when I actually realised, that all of this really happens.
Things changed after the on arrival training and as Christmas came closer, I realized something. I couldn’t write anymore. My creativity was dead. I thought it’s just a small writers block, but it was much more. Everything I wrote seemed terrible and irrational to me. I had a huge project, a play that I was supposed to present (party) around Christmas when I went back to Austria for holidays. Additionaly, I was haunted by homesickness. I’m going to be fully honest here – I became really depressed at that point. Our bosses tried everything they could to cheer me up but all of that had nothing to do with my work here. I thought about quitting. I felt emotionally deaf, I didn’t feel like myself anymore. I couldn’t grab a positive thought, I couldn’t find things I believed in previously. I changed.
When I was in Austria over Christmas, things became better. Mostly due to the company of my friends, the contact with my mother language, the amount of sunlight (seriously, the darkness completely killed me in December) and the known environment. A person that means a lot to me asked me, if I would prefer to stay here and not return to Finland. I couldn’t even think a second of what she said before my mind instantly screamed: No. I don’t want to stay. I want to go back to my second life in Finland. Up to today, I’m not sure what that was. I had so many reasons to quit. But my inner me said no. You’ll do this. And I did.
When I returned to Finland, I enjoyed everything a lot more. My work, the people here, everything. My writer block is still there and it still worries me. But I decided not to listen to it too much.
What I want to say with that shitload of text – EVS isn’t easy. Not at all. EVS has highs and lows. And the lows can be quite heavy. It doesn’t matter how nice you bosses are, how awesome your jobs are or how many fun you have (in our case a solid ten out of ten) – the lows will come. I came here to change, and I changed, in a very different way than I expected. EVS is the first time in my life to be completely alone, no friends, no family, no familiar environment or language. The people I met here were strangers to me. And exactly these circumstances force you to deal with yourself. To experience the real you. And that’s not easy either. You may find sites of you that you never had expected to find. You maybe lose yourself in these thoughts. And you maybe experience a lot of unpleased feelings and thoughts. These are the things you don’t think about when you sign the voluntary agreement – simply because you can’t imagine all that happening to you before you experience it. But still, even though I spent days in bed feeling like death himself – I wouldn’t ever give these experiences away, for nothing in the world. Challenges make you grow, in way, that can’t be written in words.
If you are a volunteer striving this blog for advice for you future EVS, or if you are the EVS after Romain and me – don’t let these downs drag you of the course. Every down passes. There are so many reasons to do an EVS, the incredible experience, the people you meet, the places you see and the new you you’ll find. Don’t let people take influence about you decisions during your EVS. It’s you experience (used this word a couple thousand times in this text now, sorry about that) and every EVS is different. EVS is one of the few things in life that you only do for yourself. Be proud of what and of who you are.
- Alex, European Volunteer.

Health on the High Seas – Mandara Spa on the NCL Breakaway
“Health is very addictive,” Michaela Deeley told me. “The way you feel once you start to feel good… you don’t want to stop.”
Michaela is the manager at the Mandara Spa on the Norwegian Cruise Line’s ship, the Breakaway, which sails weekly from New York City to Bermuda. I appreciated the opportunity to interview Michaela about how medical tourism is a growth business for the cruise industry, and a welcome on-ramp to health and wellness for passengers.
“Health on a cruise ship?” you ask. In fact, this area of the146,000 ton ship [which accommodates 3,963 passengers and 1,657 crew is hiring more staff in response to cruising clients’ growing demand for health and wellness services on-board. When it comes to consumers and medical tourism, you must think beyond bingo, binge eating, and bar-hopping.
“More passengers are flowing into the spa area looking for services,” Michaela told me. The primary demand prompting cruise ship passengers may be seeking a week of vacation of sun-tanning (using high-powered SPF to prevent sun damage, of course), dining 24×7, and casino gambling. But more people choosing to cruise are growing more health conscious, seeking opportunities to make personal health throughout the ship: from the kitchen and dining room to the fitness center and through experiencing dozens of health education classes available throughout the seven day ride on the Atlantic Ocean.
What is prompting people to come into the Spa area of the ship, when there are so many other activity options? I asked Michaela. The simple answer is that our interest in health, vitality and wellness is growing, and particularly among people over 50 who want to age well.
While some repeat passengers may know what they want after boarding the ship, most people need time to review the lengthy, descriptive spa menu. Staff are trained to ask clients about what’s motivated them to approach the spa in the first place. The client complaints and concerns cover the gamut: to relax more, to deal with pain, to boost wellness, and to be sure, to lose weight. While this may seem ironic given cruise ships’ stereotypical reputation for 24-hour buffets, unlimited drinks packages and 3 am room service, it is the case that many cruisers seek balance, participate in daily yoga classes, and order nutritious, clean food from restaurant menus. There are vegetarian and gluten-free options for people who want them among the nearly 20 dining venues.
The spa service list is split into categories, some of which are the more typical salon and spa services available on land such as facials, massages, and mani/pedicures. The Medi-Spa services are less familiar to consumers new to the concept: body correction, acupuncture, and several branded dermatological treatments. These are generally available from dermatologists working on terra firma, but rarely under one roof with all the other services offered here – including the GOSMiLE branded tooth whitening procedure.
The demographics of the passengers are important to call out: remember that this ship’s particular route travels from New York City to Bermuda. Michaela noted that the NYC clientele lead busy lives. If people can take advantage of several services over a week in one place, and while on a relaxing holiday, it optimizes peoples’ time.
Convenience is king. A diverse portfolio of services is offered in one place. And for “patients” used to being responsible for one bill for every encounter, here, it’s one bill at the end of the cruise with no sticker shock. Prices for each service are transparently shown. There are even discounts offered, Uber-style, at low-peak times such as in port in Bermuda.
The thermal suite in the spa is equipped with modern amenities that speak to techniques used long-ago in Europe. Medical tourism goes way back to the era where patients were prescribed spas in Bath, England, and Baden Baden, Germany, to take in steam baths and salts. Here on the Breakaway, there is a Salt Room, a Sauna and Sanarium, and a thalassotherapy pool of seawater, salt, and other therapeutic ingredients.
The demand for acupuncture services afloat is growing. Acupuncture has dozens of applications, most notably for pain – a topic getting appropriately more attention in mass media owing to the opioid crisis. Another health issue for which acupuncture gets growing application is for sleep. “People get used to a lack of sleep and lack of energy and they don’t know the difference,” Michaela observed.
I shared with Michaela research I’ve used in my work and discussed here in Health Populi and in the Huffington Post on the Stress in America survey from the American Psychological Association. The most recent poll, published in the midst of the autumn 2016 election season, found that 52% of Americans said the 2016 Presidential election was a significant source of stress, compromising sleep and other health issues.
Of course, exercise has been found to be a stress-reliever and sleep-enhancer, and there is no shortage of fitness classes to be found on the NCL Breakaway. Each day, the ship’s calendar (the “Freestyle Daily”) lists all the day’s activities, including fitness and health meet-ups. On Day 3, for example, there were classes meeting for “stretch & abs,” fun cardio dance, Ryde indoor cycling, body sculpt boot camp, relieving back pain with good feet, a salsa dance class, and many sporting choices from basketball and shuffleboard to scaling a rock climbing wall and doing an evening stretch.
I’ll give a special shout-out for the Ryde indoor cycling class, which is akin to Soul-Cycle-At-Sea. Indoor cycling for fitness has gained great traction on land, and so as a fan of cycling, I’m glad to see this fitness trend reach cruising so that consumers who want to try it out can do so in a relaxed, non-intimidating environment. What’s intriguing about the Ryde class is that riders wear a heart monitor so they can viscerally feel what it’s like to push their personal fitness: the monitor communicates an exerciser’s real-time activity which is digitally posted to a wall graphic, and a personal trainer can educate the rider on how hard to push. At $20, it’s a fair deal compared with prices on land.
At the end of the day (or cruise week), the big remaining question and wild card is whether a passenger on a holiday will continue keeping up a health regimen on-land, day-to-day, once normal workdays, workflows, and stressors enter back into our normal lives.
“We are realistic,” Michaela said, “not to tell someone, for example, to change their diet. On board, people have our support. When you go home, you must have your partner or friends support you. On-board, personal trainers show people how to do new exercises, weight lift routines, yoga poses, and good posture form in a relaxing, supportive environment. But behavior change is hard, so seeing repeat customers throughout the year (especially accessible for health consumers living around the New York metro area) is one way the spa staff knows they are impacting peoples’ lives.
Health Populi’s Hot Points: In the past year, since I first met up in the Breakaway’s Mandara Spa for an interview, I see the breadth of services offered expanding, and health consuming cruisers getting more savvy and demanding.
Several market forces are driving this phenomenon:
1. For American passengers boarding the ship, the growth of high-deductible health plans and out-of-pocket costs are dramatically reshaping US patients into healthcare consumers. 2. As consumers, people are seeking convenient care in formats, places, and at prices they find representing value – value, on their personal terms based on taste, culture, respect, and indeed, price. 3. Healthcare consumers are morphing into health/care consumers, re-defining what inputs are useful for their lifestyles, lives, and medical conditions. If dealing with pain, learning about and accessing acupuncture, posture, or massage may be of interest. If managing weight, a re-set over the course of a week of pampering and meditation could help one re-focus and re-commit to the objective. Ditto for sleep issues.
Michaela, who hails from the United Kingdom, has observed that American patients appear quick to get medications and surgical procedures. “Guests we see now want to look for alternatives. Today, they are more knowledgeable about options and availability” via, for example, watching TV programs like The Biggest Loser, YouTube videos featuring doctors and nutritionists, or reading blogs by health experts and would-be ones (a current example is the brewing controversy between Gwyneth Paltrow’s Goop and Dr. Jen Gunter, an OB-GYN).
As in all healthcare providers and suppliers going direct-to-consumer, people need to be educated and aware of their health risks as well as the quality of the services they choose to purchase. Caveat emptor is always advised, and a lot of research can be done ahead of time before any medical tourism choice, whether on a ship or to a foreign country.
Lest you think these services aren’t part of mainstream American healthcare, STATNews published a story in March 2017 on major U.S. medical institutions offering acupuncture, Chinese herbal therapy, massage, homeopathy, and other natural treatments. Some are shown in this graphic, part of a much larger one in the article.
As people look to slow age, enjoy vitality and more energy, and get good sleep, we’ll keep searching for services and experiences that bring those benefits. For thousands of cruise passengers boarding ships every week, these services will be available. Consumers looking to take advantage of these – especially medical procedures – can review these online in advance, and also check with health insurance plans to see if they can be covered by insurance or claimable in medical or health savings accounts.
Our THINK-Health forecasts expect more, and more diverse, opportunities in the retail health landscape for medical tourism and spas morphing closer and closer to health/care. People are seeking health where they live, work, play….and travel.
